In May of 2000, the Fire Department initiated a "Customer Satisfaction Survey" to gauge the public's perception of the Fire Department's performance relating to emergency calls for service.

If you have received service from the Fire Department and wish to offer feedback; you may download the survey and follow the instructions, or call the Fire Department Administration office at (805) 925-0951, extension 255 to have a survey mailed to you.
